  • ? Notarized Deed of Sale
  • ? Notarized Waiver of Rights – required if the transfer of Electric Connection is not stated in the Deed.

? Death of the Account Holder

  • ? Death Certificate
  • ? Proof of Succession – Consent from the surviving heirs is required if there is no will.

? Authorized Transfer

  • ? Notarized Authorization Letter
  • ? Notarized Waiver of Rights

? General Requirements and Fees

  • ? Photocopy of Proof of Ownership
  • ? Photocopy of Valid ID
  • ? Statement of Account (any month)
  • ?️ One (1) 2×2 Picture
  • ? Certificate of Pre-Membership Seminar Attended
    • ? Authorization Letter for representative (with valid ID), if applicable

Fees:

  • ? Membership Fee: PHP 5.00
  • ? Change Name Fee: PHP 44.80

? Pre-membership Seminar: Held every Monday, Wednesday, and Friday at 8:00 AM at ZAMECO II Main Office, Nagbunga, Castillejos after application approval.

  • ? Must present the original copy/s; photocopies are to be submitted with the application
  • ? All outstanding balances must be fully paid before the account transfer

ℹ️ Other Special Cases:

  • ✏️ For clerical errors (e.g. name misspelling): only a valid ID is required
  • ? For name change from maiden to married: Marriage Contract is required

? Note:

  1. Submit specific + general requirements to any ZAMECO II Office
  2. Waiver of Rights format is available at any ZAMECO II Office
  3. If the applicant attended a Barangay Meeting within the last 6 months, it may count as the Pre-Membership Seminar
